OUTPATIENT APPOINTMENT
Thank you for choosing to see me for your outpatient consultation. This letter sets out some important information that I am required by law to provide to you. This is for your information only and is not a bill. As this includes information about my charges, if you do not have private medical insurance but if someone else will be paying your bill, you may wish to pass a copy of this letter to them. Please note that even if someone else is paying your bill or you have private medical insurance, you are responsible for paying any charges which they do not pay.
Private Medical Insurance patients
I am fee assured and will always abide by the tariff set by your insurer. If you have private medical insurance, please contact your insurer before your consultation to check the terms of your policy, particularly the level and type of outpatient cover you have, including any reimbursement limits on individual consultation fees. Those limits and excess fees will be set out in your contract with your insurer.

Self-funding patients
I am fee assured and will always abide by the tariff set by your insurer. Those limits and excess fees will be set out in your contract with your insurer.

My fee for an initial consultation will not exceed £250 and my fee for any follow-up consultation will not exceed £150 which needs to be paid prior to consultation. These estimates are correct as at the date of this letter. My fees for procedures performed during an outpatient consultation will be added to the consultation fee but will not exceed the following:
· Flexible nasopharyngoscopy- £130
· Micro-suction / dewaxing of the ear- £70
· Cauterisation of the nose- £100
· Nasal packing- £75
· Biopsy of oral lesion- £80
These fees do it include the clinic or hospital fee, which vary from one place to another and will be payable directly to the facility where you have the procedure done. So please check the full price before agreeing to go ahead.
Should you cancel your appointment with less than 24 hours’ notice or not attend without prior notice, a cancellation fee of up to 20% of the consultation fee may be charged.
Following your consultation, you may need certain tests (such as blood tests, skin allergy tests, hearing tests or imaging, for example an X-ray, ultrasound, MRI or CT scan) to help me diagnose your condition. The fees will be determined by the clinic or hospital and charged to you, or your private medical insurer, separately. I do not receive any part of that fee.
Financial Interests
I am legally obliged to tell you if I have any financial interests in the any of the facilities I practice in or any equipment there. I can confirm I do not have any such financial interests.
